  • CRYPTOSPORIDIOSIS
    Sources are stool from infected people and animals, and contaminated food and water. Prevention is most important with CD4 count <200 cells/mm3. Patients with HIV should: 1) avoid contact with stool from patients with possible cryptosporidiosis and stool from pets, especially dogs and cats <6 months of age; 2) sex practices that involve oral contact with feces; 3) avoid drinking water from lakes or rivers and 4) avoid raw oysters. During outbreaks involving drinking water: boil water for 3 minutes, use bottled water (see www.bottledwater.org or call 1-703-683-5213) or put water through a 1 um filter (see www.nsf.org/notice;cryptospo.html or call 1-800-673- 8010). Note that rifabutin and azithromycin may have a protective effect. Also note that bottled water is unregulated and many commercial supplies are not different from municipal water supplies (that are regulated).

Hepatitis A Recommended if at risk (non-immune with chronic liver disease, MSM, IDU, hemophilia, or travel to endemic areas). Also, can consider in all non-immune patients without above risks.
Hepatitis B Recommended if non-immune
Influenza: FluMist (1) Contraindicated
Influenza: IM vaccine Recommended yearly; most effective if CD4 >100/mm3
MMR Recommended with exposure or travel. Contraindicated with severe immunosuppression.
Meningococcal vaccine Use if indicated
Pneumococcal vaccine Recommended when CD4 >200 cells/mm3
Tetanus-diphtheria Recommended; give booster every 10 yrs or with tetanusprone injury
Varicella (Varivax) (2) Indicated for non-immune patients (approximately 5% in U.S. are susceptible). Contraindicated with CD4 <200 cells/ mm3
Smallpox (3) Contraindicated except with exposurre
Human papilloma virus Recommended for women 13-26 years
